ResponsibilitiesAs a Home Health Registered Nurse (RN), you’ll:

• Provide compassionate, home-based care for patients recovering from illnesses, injuries, and chronic conditions
• Collaborate with physicians and multidisciplinary teams to deliver optimal patient care
• Assess patient progress, document findings, and communicate changes to the physician
• Educate patients and families on care plans, medications, therapies, and healthy living
• Coordinate the safe discharge of patients from services, ensuring all goals are met

Required SkillsWhat We’re Looking For:

• Graduate of an accredited Registered Nurse (RN) program
• Active, unrestricted RN license in South Carolina or compact multi-state license
• At least one year of nursing experience (home health care, Med/Surg, or Critical Care preferred)
