Position Description
Summary:
The RN Case Manager is responsible for coordinating patient care and ensuring effective disposition planning within the hospital. This role involves assessing patient needs, developing care plans, and collaborating with healthcare providers to facilitate timely and appropriate discharge planning. The RN Case Manager will work closely with patients, families, and interdisciplinary teams to optimize patient outcomes and ensure continuity of care. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.
The schedule for this role is M-F 8-430.
Key Responsibilities:
- Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs.
- Develop individualized care plans and disposition plans in collaboration with the healthcare team.
- Coordinate and facilitate patient discharge planning, including arranging post-discharge services and follow-up care.
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and adherence to care plans.
- Monitor patient progress and adjust care plans as necessary to meet changing needs.
- Advocate for patients' needs and preferences, ensuring patient-centered care.
- Maintain accurate and timely documentation of case management activities.
- Participate in interdisciplinary team meetings and case conferences.
- Provide education and support to patients and families regarding discharge planning and available resources.
- Ensure compliance with hospital policies, procedures, and regulatory requirements.
Qualifications:
- Current RN license in South Carolina or compact.
- Bachelor's degree in Nursing (BSN) required
- Minimum of 3 years of clinical nursing experience, with at least 1 year in case management or discharge planning.
- BLS (AHA)
- Strong knowledge of healthcare systems, discharge planning, and community resources.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work effectively in a fast-paced, multidisciplinary environment.
